Child Support - Paternity Opportunity Program (POP)

California’s Paternity Opportunity Program allows unmarried parents to establish legal paternity for their child by voluntarily signing a document called a “Declaration of Paternity.” If the mother is legally married to another man, this program is not available.

The Declaration of Paternity forms are available at hospitals and clinics at the time of the child’s birth. Signing the form in the hospital is a quick, easy, and free way to have the biological father’s name added to his child’s birth certificate. The forms are not available online.

If the form is not signed at the hospital, unmarried parents can visit their local child support agency and sign the Voluntary Declaration of Paternity (POP form). The staff member can explain the process and witness the signatures.

In addition to the birthing hospital and the local child support agency, parents can complete a POP form at the local Registrar of Births(Recorder’s office), and the Family Law Facilitator’s office.  The Siskiyou County Recorder’s office is located in the Courthouse, First Floor at 311 Fourth Street, Yreka. The Facilitator’s Office is also in the Courthouse, call 530-842-0157 for a schedule.

 If the parents cannot come together to the local child support office, then the signatures can be witnessed by a Notary Public. Notaries can be found in banks and attorney offices. There is usually a fee charged for this service.
