Reportable Diseases
Under the California Code of Regulations, Title 17 (Sec 2500), healthcare providers are required to report known or suspected cases of Title 17 diseases or conditions to the local Public Health Department.
The Communicable Diseases Team will investigate all disease reports, and provide guidance for treatment, prophylaxis, and infection control.
Reporting certain diseases may activate state and federal emergency response systems.
How to Report
Healthcare providers can report known or suspected cases of Title 17 diseases or conditions by calling (530) 841-2134 and asking for the Nurse of the Day (NOD), or by faxing the completed Confidential Morbidity Report (CMR) Form to (530) 841-4092.
