Please Note

We will mail you a copy of your library card, which may take up to one week from receipt and processing of the form. If you enter an e-mail address on the form, you will get your library card number in an e-mail as soon as your application is processed.

Patrons who are 15 years of age or younger are considered child patrons.

By filling out this form, you are agreeing to be financially responsible for fines incurred by yourself (or your child, if applicable) for overdue, lost, or damaged materials, according to the fines policy of the Siskiyou County Library.

Parents or guardians, not library staff nor volunteers, have the role to determine what their child/children may check out. Children can use the Library's public computers to access library resources and general Internet sites. The Library does use a filter to block obscene content, but it is not 100% effective.

Library cards allow access to all of the Library's online resources from any computer through the Library's website.
